Activity Assistant – Full-time opportunity available for an outgoing and creative team player with strong customer service skills to help plan and implement activities for residents across our campus, ranging from independent residents to those affected with dementia. Prior related caregiver experience preferred; activities experience also desirable. Duties will include driving a small bus or pontoon boat for activities; a current WI driver license and good driving record are required. Organizational skills to maintain appropriate records are also essential. This position is primarily day shift, Monday – Friday, but will include some evening and weekend hours. LPN – Memory Care – We are seeking an experienced LPN to guide the daily activities of staff and clients in our Memory Care CBRF. The ideal candidate will have a demonstrated collaborative approach to supervision and experience working with residents with Dementia. Guided by the Principals of Person First, this position will be involved in hiring and training staff; will maintain communications with families; and will take an active role in Admissions, Behavioral programming, and Plans of care, responding appropriately to the changing health needs of the residents. Hours will be full-time, primarily Monday – Friday, 8 am – 4:30 pm, with some evening and weekend hours; will be in weekend on-call rotation. Café Cook – This line cook position is available in our beautiful lakeside café. The position is full time, eligible for our full benefit program, with no late hours! Shifts vary between 6 am – 2:00 pm and 8 am – 4 pm. Minimum of 6 months related experience required. Assist with general café operations; prepare orders while maintaining high standards of quality food production, safety and sanitation.

Cook – Full-time – Benefit eligible position in the kitchen of our Assisted Living facility. Shift hours vary between 6:30am – 2:00pm or 12:00pm – 7:00pm. Experience preferred, but will train. If you have always enjoyed cooking, this may be the job for you! Cooking knowledge is a must.
RN – PM Supervisor – We are seeking an experienced RN with evidence of leadership skills to oversee the care of residents, primarily in long-term & rehab settings, on 2nd shift. Outstanding assessment and strong communication skills required; this position works closely with MD’s and provides guidance for staff. At only 4 shifts/week (including every other weekend), this position is eligible for our full benefit program. Home Care Assistant – Part-time positions available 8am-1pm Saturday and Sunday, 6pm-11:30pm includes every other weekend and part-time with flexible scheduling. Will provide in-home services which may include housekeeping, meal prep, transportation, med reminders, or assistance with dressing and/or grooming. Must be 21 with a valid driver’s license, good driving record and reliable vehicle. Must be able to lift up to 30 lbs., and assist with transfers of adult clients. Related experience, especially with seniors, a plus.
Household Attendant, Memory Care – Part-time, 8-noon, every other weekend. Position involves light cooking/serving meals and cleaning, with the opportunity to interact with residents in a Memory Care setting. Minimum age 18. Training provided.
Household Attendant, Health and Rehab Center – Benefit eligible opening on Days (7-3); and part-time openings Days (7-3) and PMs (4-8) including every other weekend. Position involves light cooking/serving meals and cleaning, with the opportunity to interact with residents. Minimum age 16. Training provided.
Custodian – Full-time, benefit eligible position (2:00 – 10:30pm), including every fourth weekend. Duties include floor care, housekeeping and room set-up. Must be customer service oriented. Previous experience a plus.
Housekeeper – Full-time, benefit eligible position (7:00am – 3:00pm). Performs housekeeping tasks in apartments and various locations. Serving seniors in an independent living setting.
