Sub-Acute RN/Household Coordinator – Responsible for overall management of the Rehabilitation Household; responsible for our residents’ care transition from post-acute admission to discharge. Good communication skills are necessary to help coordinate individualized care with the interdisciplinary team. RN with Rehabilitation, Management or Medical/Surgical background preferred.
Registered Nurse/Licensed Practical Nurse (RN/LPN) – Full-time PMs, part-time all shifts, includes every other weekend. May work with rehab or long-term care residents. Generous sign-on bonus for all Nursing positions!
Care Companion/CBRF – Looking for caregivers to work in Memory Care household; CBRF experience or CNA certification desirable. Minimum age 18. Benefit eligible opening Nights, Part-time Days (7-3) and PMs (3-11). $600 sign-on bonus!*
Security / Custodian – Occasional status position available, needed to fill in for vacations or sick time. Duties include light custodial and maintenance, and making rounds. Weekday hours 3:30-9:30 pm; weekends 8 am-4:30 pm; will participate in holiday rotation. Must be able to walk for most of shift and lift up to 50 lbs.; customer service orientation essential. Valid WI driver license & good driving record required. Min. age. 18; people of all ages are encouraged to apply.
Home Care Assistants – flexible scheduling, part-time. Will provide in-home services which may include housekeeping, meal preparation, transportation, med reminders, or assistance with dressing and/or grooming. Must be at least 21 with a valid driver’s license, good driving record and reliable vehicle. Must be able to lift up to 30 lbs., and assist with transfers of adult clients. Related experience, especially with seniors, a plus.
